What Happened?
Centers Laboratory says it discovered suspicious activity in its information technology environment on August 25, 2025. The company isolated the affected systems and began an investigation with the assistance of third-party forensic specialists.
The investigation determined that an unauthorized actor gained limited access to data stored on Centers Laboratory’s systems between August 9 and August 14, 2025. During that period, the unauthorized actor copied data from the company’s systems.

What Information Was Involved?
The information affected was not the same for every individual. According to Centers Laboratory, the compromised files contained names together with one or more of the following:
- Dates of birth
- Social Security numbers
- Driver’s license or state identification numbers
- Passport numbers
- Health insurance information
- Medical information
The HHS breach portal identifies Centers Lab NJ LLC as a healthcare provider and classifies the incident as a hacking or information technology incident involving a network server.
What Can Criminals Do With This Information?
Stolen personal and health information may be used to commit identity theft, open fraudulent accounts, submit false medical claims, conduct phishing attacks or impersonate affected individuals. Social Security numbers, government identification and medical information may create risks that continue long after the breach is announced.
